Most likely, the file that stores folder icon settings has been cleaned or damaged.
I have attached a file with default settings.
Edit / Project Settings / Ultimate Editor Enhancer / File / Import / Items.

At one point folder icons were working. I removed Ultimate Editor (was trying troubleshooting an editor issue and needed to rule Ultimate Editor out). After adding Ultimate Editor back to the project, I found Folder Icons no longer working after reconfiguring them. Screenshot include to confirm I have correct configuration.

Any help would be greatly appreciated!
